PICO Question Examples for Nursing (20 Examples by Question Type)

A PICO question turns a clinical problem into something you can search for. It names the Patient or population, the Intervention (or exposure), the Comparison and the Outcome. PICOT adds Time. The format comes from evidence-based medicine and was popularized in a 1995 ACP Journal Club article, "The well-built clinical question" (Richardson et al.).

Most students get stuck on two things: writing a question that's too broad to search, and using the intervention template for every question type. Below are 20 examples sorted by type, a template for each type, and the search terms that come out of each question.

The question type decides the wording, and the type of study you'll look for.

Meaning questions often don't have a comparison, so they are sometimes written as PIO or PICo (Population, phenomenon of Interest, Context).

Intervention

These examples are written to be searchable. They do not claim what the evidence shows; your literature search answers that.

From PICO to search terms

Each part of your question becomes a search concept. Combine synonyms with OR and concepts with AND.

Example question 6: hourly rounding and falls in long-term care.

Search: (P terms) AND (I terms) AND (O terms). Leave out C and T at first; add them only if you get too many results.

Tips:

  • Use subject headings as well as keywords: CINAHL Headings in CINAHL, MeSH terms in PubMed.
  • Use for truncation (fall finds fall, falls, falling).
  • Filter by date (usually the last 5 to 10 years), language and study type.
  • Canadian programs often accept guideline sources too. In Ontario, RNAO Best Practice Guidelines are a common starting point.

Frequently asked questions

What is the difference between PICO and PICOT?

PICOT adds Time: the period over which you'll measure the outcome. Many nursing programs ask for PICOT; check your rubric.

Does every PICO question need a comparison?

No. If there's no clear alternative, the comparison can be "usual care" or left out. Meaning questions usually have no comparison.

What is a PICo question?

A version for qualitative questions: Population, phenomenon of Interest and Context. It suits questions about experiences and perceptions.

Which databases should I search?

CINAHL and PubMed (MEDLINE) are the core databases for nursing. The Cochrane Library is useful for systematic reviews.

How many sources do I need for an EBP paper?

It depends on the assignment. Many undergraduate EBP papers ask for 5 to 10 peer-reviewed sources published in the last 5 to 10 years.

Can I use a PICO question for my capstone?

Yes. Most nursing capstone and evidence-based practice projects start with a PICOT question.
